Can the prognosis for ALS be determined from an EMG?
EMG is not a suitable method for assessing the prognosis of ALS. Along with other diagnostic criteria, EMG helps determine whether a diagnosis of ALS is present. However, the measurements are not quantifiable, so it is not possible to assess the prognosis using EMG. Scientific research is currently focused on developing specialized techniques for a “quantitative” EMG method designed to support prognosis assessment. Even with these experimental techniques, it is unlikely that the EMG method “alone” will be able to determine a prognosis in the future.
The most reliable method of prognosis assessment is the combination of various criteria, particularly the ALS Functional Rating Scale (ALSFRS-R), vital capacity, and the neurofilament biomarker (NfL). However, the following also applies to the aforementioned prognostic criteria: Due to the complexity of ALS, individual measurements or parameters are not suitable for determining the prognosis. Taking a wide variety of criteria into account in combination is crucial for the assessment.
